The people to whom it refers know the word well. They don’t like it.
That’s… the point. The great majority of the time it’s not being used to refer to people with actual mental disabilities. It’s usually referring to someone being stupid. If you called a person with actual mental disabilities the R-word I’d imagine most people here would have a problem with that.
